Remarks

• Above rates are valid till 31st day of December 2010 and are minimum for standard risks. Final rates to be determined after risk assessment upon receipt of completed Individual Proposal Form.
• Premium payment is annual and in advance on cash basis only.
• An additional AED 50/- shall be charged per Policy being the Policy Fee.
• All family members (Self, Spouse & Children) residing in UAE should be covered.
• Outpatient claims on REIMBURSEMENT basis (pay to the Provider and claim from OIC). Inpatient claims on direct billing basis.
• Pre-existing & Chronic conditions, Routine Dental benefits, Maternity benefits, and Routine Optical Treatment/Appliances are excluded. Other Exclusions are listed in the EXCLUSIONS list
• Member to follow Oman Insurance standard Prior Approval Protocols
• The List of Network Hospitals/Clinics/Pharmacies/Labs is available
• A coinsurance of 20% shall be applicable for claims from non-network providers.
• The benefits offered in this scheme does not comply with the Health Authority of Abu Dhabi, U.A.E. regulations for compulsory health insurance.
• To apply, fill in the attached INDIVIDUAL PROPOSAL FORM and submit to us along with valid Passport with Visa page copy and 1 passport sized Photo. The QUOTE ACCEPTANCE FORM is also required to be filled and signed to proceed with the Policy Issuance.
